Summary:
Under the supervision of the Director of Medical Operations (DMO), the Primary Care Nurse Supervisor (PCNS) leads the office primary care team to reduce, eliminate, or manage health issues affecting the health and well-being of Signature Health Patients. The PCNS participates in treatment plan development, provides assessments, treatment services and education. The PCNS adheres to all Local, State, and Federal laws, regulations, and requirements.

Responsibilities:
Leadership
• Staff Development: Selecting, orienting, training, assigning, scheduling, coaching, counseling, and disciplining assigned staff; communicating job expectations; planning, monitoring, appraising job contributions; and adhering to policies and procedures
• Primary Care Medical Assistants (MAs), Licensed Practical Nurses (LPNs), and/or Primary Care Receptionists report to the PCPS by work site.
• Schedules: plans work schedules and delegates responsibilities and tasks in conjunction with DMO
• Enforces adherence to State, Local, Federal, and to other governing agency regulations;
• Works within budgetary requirements& compliance
• Participates in all assigned staff meetings and staff development programs
• Assists in community outreach
• Maintains clear communication with co-workers, referents or designated liaison person. Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care plans
• Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures
• Provides oversight of logs and required checks (i.e. refrigerator temperatures, emergency medications, expired medications, oxygen, testing controls, etc.)
• Liaison to Site Leadership and other Signature Health Programs
Medical
• Meets medical operational standards by contributing information to strategic plans and reviews; implementing productivity, quality, and customer-service standards; resolving problems; and identifying system improvements.
• Assures high quality of care by use of Primary Care standards of care and evidence based practices; measuring health outcomes against standards; making or recommending adjustments
• Ensures operation of medical and administrative equipment by verifying equipment availability; completing preventive maintenance requirements; following manufacturers instructions; troubleshooting malfunctions; calling for repairs; maintaining equipment inventories; evaluating new equipment and techniques
• Maintains vaccine and supply inventory by studying usage reports; identifying trends; anticipating needed supplies; ordering and receiving supplies.
• Coordinates vaccine programs and inventory with Vaccine Coordinator (VC)
• Develops and facilitates health and disease patient education, including leading group/shared office visits
• Conducts comprehensive, preventive screenings for patients
• Draws, collects, processes and stores bodily fluids for Point of Care (POC) and 3rd Party testing
• Provide back up to Medical Assistant for direct patient care and assistance to Providers.
Quality and Compliance
• Assists patients through the healthcare system by acting as a patient advocate and navigator by coordinating continuity of patient care with external healthcare organizations and facilities, with patients and families following hospital admission, discharge, and ER visits
• Participates in team meetings and in all Quality Assurance and Utilization Review activities
• Supports patient self-management of disease and behavior modification interventions
• Utilizes SH Disease Registries, coordinates high risk patient care with Providers, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting
• Completes medical record review to assess documentation quality and billing of all services provided
• Participates on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the FQHC initiatives
• Maintains client charting in accordance with FQHC, CLIA, OSHA, OARRS, TDDDL, Medicaid, Medicare, and other third party professional standards
General
• Secures patient information and maintains patient confidence by completing and safeguarding medical records
• Maintains safe, secure, and healthy work environment by establishing and following standards and procedures; complying with legal regulations to maintain proper clients rights and confidentially
• Updates job knowledge by participating in educational opportunities; reading professional publications
• Enhances practice reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments
• Participates in all assigned staff meetings and staff development programs
• Contributes as a team player that can follow systems and protocols to achieve a common goals and align with SH Mission
• Maintains clear communication with co-workers, referents or designated liaison person
• Participates in miscellaneous duties and projects that may be assigned from time to time
• Maintains professional licensure

Responsibility and Decision Making Authority:
Responsible for working collaboratively with physicians and multidisciplinary team members to make decisions concerning patient treatment. Promote and restore patient’s health for patients of all ages, genders, cultures, and backgrounds; provide physical and psycho-social support to patients, friends, and families. Provide patient care within the scope of Ohio State RN licensure.

Minimum Requirements:
• Current, valid RN licensed to work in the state of Ohio
• 2-5 years’ experience in clinical setting
• Ability to effectively communicate both in writing and orally
• Must be highly organized and demonstrate sound judgment with decision-making and problem-solving skills
• Proficient in Microsoft Office and Electronic Health Records
• Self-disciplined, energetic, passionate, innovative
• Able to continuously and/or frequently preform grasping, fine dexterity, balancing, standing, reaching, sitting, lifting 25lbs.

Preferred Qualifications:
• A working knowledge of mental illness and the disease concept of chemical dependency
• Leadership and Team Management
• Working knowledge of Primary Care Medicine and Practice, Physical Health Patient Education, Diabetic Management, and/or Wellness activities
• Phlebotomy certified with experience with routine exposure to blood or body fluids
• Bilingual, preferably Spanish speaking.
