AllHealth Network is growing! We are seeking a motivated individual to be a part of our Assertive Community Treatment team in the FULL-TIME position of Case Manager to work in Douglas County area. If you are passionate about what you do and want to make a difference, please apply.\r
Founded in 1955, AllHealth Network provides professional, culturally inclusive mental health and substance abuse services, including outpatient services such as individual, family, marital and group counseling and psychiatry plus a full range of specialized programs including an acute treatment unit, program for those with severe and persistent mental illness, and more.\r
Case Manager\r
Position Summary:            \r
Assist individuals and families with increasing community support, resources and coping skills through care coordination, resource linkage and counseling to help clients' reach their recovery goals.\r
Education: Master's degree in Criminal Justice.\r
Experience: Minimum of 2 years' experience working in a clinical setting in the behavioral health field. \r
Demonstrate a high level of skill in assessment, evaluation and therapeutic engagement of diverse clients; use of evidence-based counseling models and techniques that match presenting problems; good diagnostic skills and treatment\/care planning; and the ability to work with clinical documentation and electronic health records (EHR). \r
Knowledge of DSM V and working principles of recovery and trauma-informed care in treatment planning and in direct care. Experience working with individuals and families who experience various mental, behavioral, and substance abuse conditions. \r
Ability to operate PC-based software programs including proficiency in Word, Excel and other computer based systems. \r
Possess advanced written and oral communication, decision making and problem solving skills, and efficient time management. Excellent interpersonal skills, the ability to work independently and as part of a team; collaborating with other internal and external teams. \r
Must possess own transportation, have a good driving record, a valid driver's license, and proof of insurance. \r
Collaborate within a team-based model to ensure effective and efficient delivery of care coordination and case management services. Includes telephonic and face-to-face contact; interventions are provided in the community and in the office. \r
Complete Case management assessments to identify individual needs and develop a case management plan. \r
Work collaboratively with clients and their identified support systems to develop a strengths based mutually agreed upon care plan. Update the care plan as needed and no less than every 6 months. \r
Conduct safety assessment and planning for clients as indicated. Utilize the M1 process when needed. \r
Act as a client advocate and a liaison between internal and external providers, and community agencies to seamlessly integrate and coordinate services. \r
Provide clinical interventions that are consistent with the client's readiness to change and engagement in services. \r
Make referrals to community based organizations and resources that are consistent with the identified needs in the plan of care and\/or by the client\/family. \r
Conduct treatment plan review and care coordination with other providers\/community agencies on the client\/families' treatment team.\r
Positively contribute to the financial health of the organization by ensuring delivery of billable services and utilizing, monitoring and collecting data for decision making and program improvements. \r
Maintain a positive, ethical, and professional working relationship with community, government agencies, and other professionals. \r
Participate and contribute to departmental meetings, team huddles, rounds, supervision and other meetings, as needed, to ensure cohesive department operations. \r
Enhance professional growth and development through participation in specialized training. \r
Perform case management services consistent with ethical and professional standards and disciplines pertinent to the employee. \r
Follow all AllHealth Network policies and procedures.\r
Complete all required trainings as listed in Relias Learning (both online training and face-to-face training) within required timelines.\r
Perform other duties as required within the scope of the position and the experience, education and ability of the employee.\r

Founded in 1955 as Arapahoe Mental Health Center, we became a Comprehensive Community Mental Health Center as a result of legislation on July 1, 1967 and two federal grants. In 2000, Arapahoe Mental Health Center implemented the doing-business-as (dba) name of Arapahoe Douglas Mental Health Network (ADMHN). Today ADMHN is responsible for mental health programs serving residents of Arapahoe County (not including the City of Aurora) and Douglas County. Many programs are open to residents outside this area, including the insurance-only Turning Points, A Counseling and Psychiatric Practice.
