The RN-Clinical
Documentation Specialist facilitates accurate, complete, consistent and timely
documentation for severity of illness and quality in medical records completed
in the physician office setting. This involves extensive record review,
interaction with physician/non-physician providers, health information
management professionals, nursing staff, and other physician office staff. Active participation in team meetings and
education of providers and staff is required.
ESSENTIAL FUNCTIONS:
Review medical record(s) for completeness, accuracy, consistency
and clinical evidence for severity of illness (appropriate capture of true
disease burden) and quality using official RA (Risk Assessment) and other relevant official
documentation/coding guidelinesAccurate, comprehensive and timely medical record reviewConsistent and effective use of Compliant Documentation
Management Program® (CDMP) RA strategies Recognize opportunities for documentation improvementFormulate clinically credible documentation clarificationsRequest documentation clarifications as appropriate for severity
of illnessEffective and appropriate communication with
physician/non-physician providersTimely follow up on all cases and resolution of those with
clinical documentation clarifications Provide routine formal and informal (just-in-time) RA and
documentation improvement education to providers/staff Participate in RA steering committee meetingsManage and adjust to
changing work volumes and prioritiesCommunicate with HIM and/or billing staff and resolve
discrepanciesDocument activity and outcomes accurately, consistently, and in
timely mannerParticipate in data collection, analysis and reporting for
program management and performance
improvement
','!*!MINIMUN KNOWLEDGE,
SKILLS AND ABILITES REQUIRED:
Current license as a registered nurse issued by
the State of Iowa or compact stateBachelor’s degree in nursing preferredMinimum three years nursing experience required,
5 years preferredUnderstand and communicate differences between
Medicare Part A and B and RA payment methodologies [i.e., CMS Hierarchical
Condition Categories (HCCs), RX HCCs, HHS HCCs, etc.] and how documentation
impacts HCC capture and assignment (upon completion of training)1 year of Clinical Documentation Improvement
(CDI) experience in the inpatient setting
preferredProficient in use of computer applications, i.e.
MS Office programs.
